
Youth Pathways
The objective of Youth Pathways is to assist the most at-risk
young people make a successful transition through to completion
of Year 12 (or its equivalent) and ultimately, to further
education, training or employment and active participation
in the community.
The priority outcomes for participants of Youth Pathways are:
1. retain or re-engage participants in the school environment
through to completion of Year 12 or its equivalent;
2. engage or re-engage participants in non school-based education,
vocational learning and training; and
3. prepare participants for effective engagement in Job Network
activities.
To be eligible to participate in Youth Pathways a young person
must be an Australian Citizen or permanent resident between
the ages of 13 to 19 and either:
1.
a) at risk of leaving school before completing year 12; and
b) at risk of not making a successful transition through school
and/or further education, training or employment;
OR
2.
a) have left school in the preceding 12 months without completing
year 12; and
b) at risk of not making a successful transition to further
education, training or employment.
For the purposes of Youth Pathways a young person who is at
risk of leaving school before completing year 12 and of not
making a successful transition to further education, training
or employment is defined as typically, having multiple barriers
that limit their opportunities to participate in education,
training or employment.
Nationally, Youth Pathways will assist over 17,000 of the
most at-risk young people each year. There are 100 Youth Pathways
service regions with one provider delivering services per
region. The service model for Youth Pathways consists of three
phases - Assessment Phase, Assistance Phase and Ongoing Support
and Guidance Phase. A summary of what providers will be required
to provide at each phase of the service model is outlined
below.
Assessment Phase:
" conduct an assessment to confirm the young person's
eligibility through the identification of barriers and the
young person's level of risk of not making a successful transition;
" develop a Youth Pathways Transition Plan setting out
the strategies to overcome barriers to a successful transition
and the assistance to be provided to the participant through
the programme.
Assistance Phase:
" deliver tailored, individualised case management and
intensive support, building upon the Assessment Phase consistent
with the participant's needs and strategies to overcome barriers,
as detailed in the Transition Plan.
" provide assistance that aims to facilitate and support
the participant to remain engaged in school or re-engage in
some form of education or training. For participants still
at school successful tenderers must provide assistance that
is complementary to the services delivered or facilitated
by the school, the LCP and other organisations.
" minimum of six hours of face-to-face assistance no
greater than four weeks apart meeting the needs of individual
participants. Each face-to-face session during this phase
is to include a review of progress against the Transition
Plan and agreement to the follow up actions.
" regular contact will be maintained - at least once
every two weeks to follow up on the activities and outcomes
of the previous session, to review progress and touch base
on the next steps.
" refer participants to relevant specialist support services
where appropriate.
" the Assistance Phase is to be delivered until significant
progress has been made to address barriers and assist the
participant to move towards desired outcomes.
Ongoing Support and Guidance Phase:
" continue with the regular contact established during
the Assistance Phase through face-to-face contact, telephone
discussion, email and text messaging. This must be no more
than four weeks apart.
" ensure that the Youth Pathways participant remains
on track to a successful transition by providing ongoing support
and guidance until the end of the calendar year, including
during school holidays.
" a participant survey will be used to gather information
on the participant's destination immediately upon exiting
the programme.
